An employee was exposed to a fall hazard, of nine stories, where their
lifeline was not rigged
independent of the transportable counterweighted outrigger used to support
the employee while
washing windows. Also the outrigger was tied back to a fence located on
the ground with a knot.
A feasible abatement method includes, but is not limited to, ensuring that
rope descent systems,
transportable counterweighted outriggers, and personal fall arrest systems
are installed and used
in accordance with ANSI/IWCA I-14.1.CA
