OSH ACT of 1970 Section (5)(a)(1): 29 CFR Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall and struck-by hazards from improperly erected suspended scaffolding.      Jobsite:  roof of connector between Emerson and Pratt Buildings.    Instance A:  A VersaBeam outrigger was not tied back.    Instance B:  A second VersaBeam outrigger was tied back to the first outrigger.    Instance C:  The tie-back on the second VersaBeam outrigger had too much slack.    Instance D:  A vertical lifeline was tied back to it's own outrigger.    Instance E:  A second vertical lifeline was tied back to the first outrigger.    Instance E:  Employees were inadequately trained in suspended scaffolding safety.    Instance F:  Rope grabs were used inappropriately.
