Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that,
employees were exposed to harmful levels of carbon monoxide while working
with and
around the VDT unit:
a)  On or about August 29, 2008 and at times prior, employees working with
and
round the VDT unit were exposed to elevated levels of carbon monoxide.
Among other methods some feasible and acceptable abatement methods to
correct this
hazardous condition would be to implement engineering and administrative
controls to
ensure employees are not exposed to harmful levels of carbon monoxide such
as:
1) Train employees in how to recognize carbon monoxide and what actions are
to be taken when elevated levels of carbon monoxide are identified.
2) Install at employee work areas carbon monoxide detectors that have
effective warning systems.
3) When carbon monoxide leaks are detected, operations need to be shut
down until repairs are made.
4) Develop a procedures for the event of elevated levels of carbon
monoxide.
Procedures should include; who is to be notified, where do employees
muster,
shut down procedures, personal monitoring, at what level of carbon monoxide
do the employees evacuate, when is it safe to return to the work area.
