On or about April 24, 2012, and at times prior, employees were exposed to struck by hazards from personnel operating powered industrial trucks in a manner which limited their forward view. On April 24, 2012, an employee was fatally injured when struck by a powered industrial truck being driven with the load obstructing the forward view.
