Section 5(A)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish to each of his employees employment and a
place of employment
which were free from recognized hazards that were causing or were likely
to cause death or
serious physical harm to employees in that employees were exposed to
crush-by hazards:
On or about 07/08/2008, employees were exposed to the hazard of being
crushed while working
inside the basket of a truck mounted boom lift that was not inspected in
accordance with the
operator's manual of the equipment.
Among other methods of abatement, one feasible and acceptable method to
correct the hazard
is to inspect the ring gear bolts and nuts as required by the manufacturer
of the equipment.

REQUIRED FOR THIS VIOLATION.
