furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of fire and/or explosion:
a) Truck # 9792 at Pazzazz Kennels - an employee was severely burned while
delivering
propane using a damaged delivery hose, that was reported as damaged by the
driver for
six working days, but the hose was not immediately replaced or removed as
soon as
reported by the driver, on or about 3-06-09.
Among others, feasible and reasonable means of abatement are to: comply
with the National Fire
Protection Association, NFPA 58 Standard for the Storage and Handling of
Liquefied Petroleum
Gases, and the Amerigas Entry Level Driver Program Topic 4.0 Bobtail
Maintenance and
Inspection, and ensure: the existing gas delivery equipment maintenance
program is fully
implemented and maintained at all locations, gas delivery equipment is
kept in a safe and proper
operating condition at all times, and ensure correct processing and use of
daily driver inspection
reports.
