furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to a struck-by hazard:
a) Lick Creek Road & Rt. 87/Cherry Mill, PA:  Cement forms were lifted
without
the use of lifting brackets, supplied by the manufacturer for that
purpose. The form
slipped off a hook, striking the employee in the head and fracturing his
skull, on or
about 08/31/09.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
ABATE THIS HAZARD IS: to follow the operating practices set forth in the
ANSI/ASME
B30.9-1984 safety standards for cableways, cranes, derricks, hoists,
hooks, jacks and slings,
Section 9-1.9(e) Operating Practices, page 18 which states: The sling
shall
be hitched or
rigged in a manner providing control of the load, OR
follow the manufacturer's recommendations by using "lifting brackets"
supplied by the
manufacturer at the time of purchase of the forms.
