Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the overhead hoist, below-the- hook lifting devices or crane system fail: a)Production Room, on or about 1/15/09: Two below-the-hook spreader beam type lifting devices attached to a Coffing 1/2 ton hoist and a CM Lodestar 1/2 ton hoist had no rated load capacity markings. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1.Mark the rated load of the lifting device on the main structure or equivalent in accordance with ASME B30.20-1993, Section 20-1.2.1. b)Production Room, on or about 1/15/09: The upper limit switch on a CM Lodestar 1/2 ton overhead hoist was not functioning. Among other methods, feasible and acceptable abatement methods to correct this hazard are to: 1.Adjust the upper limit switch in accordance with ANSI B30.16-1993, Section 16-2.3.3. 2.Conduct frequent and periodic inspections of the overhead hoists in accordance with ANSI B30.16-1993, Section 16-2.1. c)Production Room, on or about 1/15/09: Two rolling hoist carrier cranes with a Coffing 1/2 ton overhead hoist and a CM Lodestar 1/2 ton overhead hoist had no rated load capacity markings on them. Among other methods, one feasible and acceptable abatement method to correct this condition is to: 1.Mark the rated load capacity on the crane device. Abatement certification must be submitted for these items.
