On or about July 31, 2024, employees performed servicing activities such as removing grinder parts for cleaning, and cleaning and sanitizing equipment including but not limited to the poultry and beef Wielers Grinders, when the equipment was not isolated from the energy sources and rendered inoperable. The employer did not ensure procedures were developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in servicing and maintenance of the Wielers Grinders.

On or about July 31, 2024, employees performed servicing activities such as removing grinder parts for cleaning, and cleaning and sanitizing equipment including but not limited to the poultry and beef Wielers Grinders, while the equipment was not isolated from the energy sources and rendered inoperable. The employer did not ensure that each worker instructed to perform servicing tasks were trained to recognize applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.

The employer did not ensure that the equipment was locked out to protect workers from the unexpected energization, startup or release of stored energy.
