were exposed to THE HAZARD OF BEING STRUCK-BY:
a)Spraying area, on or about 09/17/2009: A Yale, overhead hand chain
operated hoist with no capacity rating had a broken safety latch on the
hook,
and no inspection, or maintenance records were being maintained.
Among other methods, several feasible and acceptable abatement methods to
correct
this hazard are to:
1.Test and rate the hoist, repair or replace the safety latch ANSI
B30.16.-1993,
Overhead Hoists, Section 16.2, Inspection, Testing and Maintenance.
2.Conduct frequent and periodic inspections of the hoists and maintain
records
or use an external coded mark in accordance with ANSI B30.16-1993
Overhead Hoists, Section 16-2.1 Inspection.
b)Spraying area, on or about 09/17/2009: A wall supported Jib rail on which
the Yale Overhead hand chain operated hoist was mounted had not been
initially tested, rated and marked to determine if it would withstand the
forces imposed by the hoist and the rated load.
Among other methods, several feasible and acceptable abatement methods to
correct
this hazard are to:
1.Test, rate, and mark the jib rail to ensure it will withstand the forces
generated by the hoist and the rated load. ANSI B30.11.1-1980, Monorails
and Underhung Cranes, Section 11.1 Construction and Installation.
2.Conduct frequent and periodic inspections of the wall supported jib rail
and
maintain records in accordance with ANSI B30.11.2-1980 Monorail and
Underhung Cranes, Section 11-2 Inspection, Testing, and Maintenance.
Abatement certification must be submitted for these items.
