Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY, and FALLING: a)On or about 12/14/09, Tower Site: Communication tower stability was not maintained during maintenance operations. Among other methods, feasible and acceptable abatement methods to correct this hazard is to: 1.Utilize temporary bracing, and/ or temporary guy wires, prior to the removal of permanent guy wires, as is addressed in ROHN Tower Safety Information, and to ensure stability as described in the Tower Climbing Safety manual, second edition and in ANSI/TIA-222-G-2005, Structural Standard for Antenna Supporting Structures and Antennas, revised on January 1, 2006, section 16.0, Installation. 2.Utilize the services of a Professional Engineer to determine structural stability prior to the removal of guy wires. Abatement documentation must be submitted for this item.
