Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to possible fire, explosion or deflagration injuries in that the wood flour dust generated was not collected to prevent accumulation: Sawdust Storage/Grinder (Scalper) Area - The chain conveyor on the sawdust conveyor bin was not provided with warning signs, personnel barriers or guarded by location. Among other methods, a feasible abatement method to correct this hazard is to guard, cover or prevent access to the lower half of the chain as indicated in Section 6.3 of the American National Standards Institute (ANSI) ASME B20.1, Safety Standard for Conveyors and Related Equipment document.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to possible fire, explosion or deflagration injuries in that the wood flour dust generated was not collected to prevent accumulation: Throughout The Facility - Employees who were working in areas served by wood dust collection systems, such as cyclones, were exposed to fire and/or explosion hazards due to the design, construction and location of the systems. Among other methods, a feasible abatement method to correct this hazard is to follow the procedures in the NFPA document 664, "Standards for the Prevention of Fire and Explosions in Wood Processing and Woodworking Facilities."
