were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to struck by injuries:
a)  Lake Orienta Elementary School Site - An elevated scissor lift was
operated in reverse and struck a
bar joist, causing a fatal injury to the operator, on or about July 10,
2008.
Among other methods, one feasible and acceptable method to correct this
hazard is to ensure that scissor lifts
are not moved while elevated in an area with overhead obstructions.
