Health Act of North Carolina:
The employer did not furnish each of his employees conditions of
employment and a place of employment free
from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in
that employees were exposed to: Caught by hazard with moving metal belt
links. [REF (ASME B20.1-2000)
Safety Standard for Conveyors and Related Equipment Chapter 5.11.2(c)(2)
The  emergency stop device shall
act directly on the control of the conveyer concerned and shall not depend
on the stopping of any other
equipment.]
a)panel glue conveyor - the emergency stop switches for the clamp operator
and the board loaders
either did not function or had to be constantly depressed for the stop
function to work.
Abatement Note :
One possible means of abatement would be to establish a pre-shift safety
inspection
on the equipment
and replace defective equipment such as emergency stop buttons that fail
to operate correctly.
