29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices for employees and contractor employees to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: a. 433 Hydrofluoric Acid (HF) Alkylation Unit, Near Pump P12C - Two carpenters were exposed to a mixture of hydrocarbons and hydrofluoric acid, after an Instrument Technician had manually opened and closed an emergency block valve. The employer did not completely implement their safe work practices and operating procedures. They did not adequately secure two lever-activated valves, energy isolating devices. All of the hydrofluoric acid in the piping system was not completely purged and neutralized. A bleeder blind flange was missing a bleeder valve and a bleeder plug. The levers that controlled these valves were not adequately secured or restrained to hold these levers in a safe or off position and physically prevent their operation. An employee was able to use these levers to both open and close emergency block valve HV-515. Observed On or About January 16, 2010. ABATEMENT NOTE: ABATEMENT DOCUMENTATION IS REQUIRED.

29 CFR 1910.119(f)(4): The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in each covered process consistent with the process safety information: a. 433 Hydrofluoric Acid (HF) Alkylation Unit - The employer failed to follow Safety Standard #20, "Safe Work Permit R and S", the refinery's established and documented general work permit procedure. When two Instrument Technicians were disconnecting several control valves and stroking several emergency block valves, General Permit # 180030 did not identify the stroking of these emergency block valves as part of the scope of work covered by the permit. Observed On or About January 16, 2010 Sunoco, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard, which was contained in OSHA Inspection 310716386,Citation # 1, Item # 10 and affirmed as a final order on February 22, 2008, with respect to a workplace located at the Philadelphia Refinery. ABATEMENT NOTE: ABATEMENT DOCUMENTATION IS REQUIRED.
