29 CFR 1910.119(l)(5): A change covered 29 by CFR 1910.119(l) resulted in a change in the operating procedures or practices required by 29 CFR 1910.119(f), and such procedures or practices were not updated: a) 22 Boiler House/Facility - On or about 10/21/10, employees who perform or are affected by emergency operations for fuel and steam balancing were without current and updated emergency procedures that included the shutdown and unavailablity of 22 Boiler House. Procedures that were not updated to reflect 22 Boiler House shutdown include RS-PH-OPS-PRO-FSS-005A, - 005B, -005C and -006A. Boiler House 22 is a PSM covered operation as a utility interconnected with a covered process and which can affect the release of a highly hazardous chemical. Sunoco, Inc., was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard, which was contained in OSHA Inspection Number 310716386, Citation Number 1, Item Number 4c and affirmed as a final order on 1/22/2008 with respect to a workplace located at the Philadelphia Refinery. 29 CFR 1910.119(g)(1)(i): Each employee presently involved in operating a process, and each employee before being involved in operating a newly assigned process, was not trained in an overview of the process and in the operating procedure as specified in paragraph (f) of this section. The training shall include emphasis on the specific safety and health hazards, emergency operations including shutdown and safe work practices applicable to the employee's job tasks. a)Jobsite: NYOM Propane and Propane/Propylene Terminal - On or about 9/2/10, an employee was performing railcar switching and spotting without training in the operating procedure, specific safety and health hazards, emergency operations including shutdown and safe work practices applicable to the employee's job tasks for a PSM covered process. Sunoco, Inc., was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard, which was contained in OSHA Inspection Number 314226903, Citation Number 1, Item Number 1 and affirmed as a final order on 8/25/2010 with respect to a workplace located at the Marcus Hook Refinery.
