Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation injuries: Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a) Southwest Steel Processing, LLC worksite located at 4900 Lighthouse Drive, Newport, Arkansas: On or about October 22, 2008 and at times prior thereto, the employer did not ensure the foot pedal control located on the working side on the Ajax Mfg. 1000C, Machine #5234P 1000 ton press was protected from unintentional actuation. Abatement Note: One feasible means of abatement is to comply with the ANSI B11.2- 1995 Hydraulic Power Presses- Safety Requirements for Construction, Care and Use, Section 6.3.9(4) which requires foot pedals to be protected (b) Southwest Steel Processing, LLC worksite located at 4900 Lighthouse Drive, Newport, Arkansas: On or about October 22, 2008 and at times prior thereto, the employer did not ensure proper and adequate operator training was provided to operators operating the Ajax Mfg. 1000C, Machine #5234P 1000 ton hydraulic power press. Abatement Note: One feasible means of abatement is to comply with the ANSI B11.2- 1995 Hydraulic Power Presses- Safety Requirements for Construction, Care and Use, Section 4.4.2 which requires the employer to be responsible for the training of personnel operating hydraulic press production systems to ensure their competence.
