29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about October 23, 2012, and times prior to, at the faciliity located at 4900 Lighthouse Drive in Newport, Arkansas, a Lockout/Tagout Program to include machine specific isolation procedures had not been developed for employees required to service and/or perform maintenance on equipment such as but not limited to the press machines and the Robot system. Failure to do so exposed employees to struck-by and caught in hazards if the machines and/or equipment were unexpectedly energized.
