falling object protective structure (FOPS) and/or other parts of the
forklift in the event
the forklift were to tip over.

industrial
truck was used to lift an employee on a work platform that was not
attached to the
powered industrial truck.  This condition exposed employees to the hazard
of falls from
elevation and being struck by the platform.
