General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by unsupported sections of concrete formworks: a) West exterior side of bridge. An employee suffered severe injuries when he fell 32- feet from an elevated walking/working platform. The employee used a pry bar to break the bond between the concrete and four connected sections of forms. The sections of the formwork had not been attached to the front-end loader or stabilized in any other manner before the employees removed all of the ties, bolts, fasteners, and/or braces. As the bond between the forms and the concrete broke four sections of formwork toppled toward the working platform forcing the employee over the guard rail system. Among other methods, feasible and acceptable methods to correct the hazardous condition may include: Securing the formwork to the front-end loader before the employees removed all of the ties, bolts, fasteners, and/or braces. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
