29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about August 2, 2013, at Project MSL00-0004-00(086), Hwy 316 and Buford Drive, Lawrenceville, Georgia: Employees setting concrete forms on a bridge cap were exposed to a fall hazard of approximately 14.2-feet when they worked outside the guard rail without fall protection.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high. On or about August 2, 2013, at Project MSL00-0004-00(086), Hwy 316 and Buford Drive, Lawrenceville, Georgia: Employees setting concrete forms on a bridge cap were exposed to a fall hazard of approximately 14.2-feet when the midrails were removed from the guardrail system.
