were exposed to crushing or struck-by hazards:
In the following instances, enclosures for the belt or rope supported
counterweights were
not provided to protect personnel from crushing or struck-by hazards:
a)Old Screen Room - The employees were exposed to the belt supported
counterweight on the Hog Fuel Conveyor.
b)Old Screen Room - The employees were exposed to the belt supported
counterweight to the Accepted Chip Conveyor.
c)Chip Handling Yard - The employees were exposed to the belt supported
counterweight to the River Belt Conveyor.
d)Pulp Dryer, Backside of #176, Tending Side - The employees were exposed
to
the rope supported counterweight for the rope sheaves.
Abatement Note: Among other methods, one feasible and acceptable method to
correct
the
following hazards is to enclose the counterweights as referenced in ANSI
B20.1-2006, Section
5.8 - Counterweights.

were exposed to avulsing, friction burns, fracturing or crushing hazards:
Waste Water Treatment Plant - The emergency stop cable for the Waste Water
Treatment
Conveyor was not taut.
Abatement Note: Among other methods, a feasible and acceptable method to
correct the hazard
is to incorporate routine inspections and corrective maintenance measures
for the emergency stop
cable systems into a maintenance program and safety and health program.
