Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards of being struck by equipment under tension
a) Williams Crusher/Pulverizer area; On 03/17/09 employees were using a
metal pipe
commonly known as a "cheater bar" to provide additional leverage on a 3/4
ton U.S capacity
Coffing Lever Hoist come-along to attempt to open the front cover to the
300 series type "k-2"
reversible impactor serial #20346 . This practice exposed employees to the
recognized hazard
of being struck by equipment should the cheater bar overload the
come-along and slings.
Among other methods, feasible and useful, one method to abate this hazard
would be to:
Discontinue the use of the "cheater bar" on a come-along and insure all
system components used
in opening the covers are in preoper working order. The Pulverizer has a
system to open the
hatch which should be maintained in working order.
b) Williams Crusher/Pulverizer Mill area; On 03/17/09 employees untrained
in the use of a
manually lever operated chain hoist were using a 3/4 ton U.S capacity
Coffing Lever Hoist
come-along with a "cheater bar" and anchored to a DBI SALA Tie-Off Adapter
SN #1003000
(used for fall protection) and a nylon web sling measuring approximately
1" x 45 " to pull open
the front cover of the reversible impactor, exposing employees to the
hazards of being struck
by equipment should the cheater bar overload the come-along and slings.
Among other methods, feasible and usefule, one feasible and acceptable
abatement method to
correct this hazard would be: 1) Train employees to be familiar with the
operating controls of
the hoist, the warnings of the hoist, the hoisting practices listed in
Section 21-1.7.2 of ASME
B30.21-1994 as Addenda to ASME B30.21c-1998, and the operator's manual
provided by the
hoist manufacturer. 2) Do not use fall protection equipment for pulling
and/or hoisting.
c) Williams Crusher/Pulverizer area; On 03/17/09 employees used a DBI SALA
Tie-Off Adapter
nylon web sling run through a rough opening on the front cover of the
reversible impactor to
anchor a 3/4 ton U.S capacity Coffing Lever Hoist come-along.
Subsequently, the DBI SALA
Tie-Off Adapter nylon sling failed under the pulling pressure causing the
front cover of the
reversible impactor to fly back and expose an employee to the hazards of
being struck by the
cover and caught in the opening.
Among other abatement methods, feasible and useful, one feasible and
acceptable method to
correct this hazard would be: Implement an inspection procedure for
inspection of damage,
proper type of slings, capacity ratings of the slings, and prohibition of
running nylon slings over
rough edged surfaces unless the rough edged surfaces are padded to protect
the slings.
***Certification and Documentation of Abatement Required***
Pursuant to 29 CFR 1903.19(d), the employer must provide documentation
that this violation
has been abated within 10 calendar days of the date it receives this
citation.
