Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to dust deflagration, explosion or other fire hazard: a)Production Area, Roasters 1 through 6: Each roaster conveys chaff from the roasting process through its adjacent hot chaff cyclone to its conveyance line which joins the Main Chaff pipeline to Green Bean Room. Each line is provided optical (infrared) spark detectors just after the cyclone but do not automatically actuate a means to control or extinguish once a hot or glowing particle is detected on or about October 20, 2009. ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement method to correct the hazard is to comply with the National Fire Protection Association 61 (NFPA 61 2008) "Standard for the Prevention of Fires and Dust Explosions in Agricultural and Food Processing Facilities." b)Chaff Conveyance System, from each roaster to the Pellet Machine: Pneumatic conveying system did not provide the air volume and velocity necessary to keep the duct interiors clean and free of residual material, on or about October 21, 2009. ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement method to correct the hazard is to comply with the National Fire Protection Association 654 (NFPA 654 2006) "Standard for the Prevention of Fire and Dust Explosions from the Processing and Handling of Combustible Particulate Solids." c)Processing Areas: Effective isolation devices to prevent deflagration propagation were not provided between pieces of equipment connected by ductwork allowing a spark or hot ember to continue to the chaff hopper, on or about October 21, 2009. ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement method to correct the hazard is to comply with the National Fire Protection Association (NFPA 654 2006) "Standard for the Prevention of Fire and Dust Explosions from the Manufacturing, Processing, and Handling of Combustible Particulate Solids"
