recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that on or
about July 7, 2009, employees were exposed to the recognized hazard of
fire and explosion from
a runaway reaction in the Ethyl Acrylate Storage Tank 2310.
Among other methods, one feasible and acceptable abatement method to
correct
this
hazard is to ensure that a shortstopping inhibitor (i.e. phenothiazine) be
designed and
utilized as an emergency response system.
