On or about April 29, 2009, at 9301 Frazier Pike in Little Rock, Arkansas,
the
employer did not ensure that the established crane operation procedure
which
required employees to stay within 20 ft. of the crane when moving a load
was
being followed.  An employee in the Spiral department was observed walking
approximately 100 ft. in front of a 25 ton steel coil that he was moving
northward
while operating the controls to a P&H overhead gantry crane that was used
to lift
the coil, exposing employees to struck by hazards.
