furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a possible entrapment of the head and torso between the
forklift and the floor
or other surface while operating the forklift should a tip-over of the
forklift occur.  One feasible
and acceptable abatement method to correct this hazard is to provide an
operator protection
(restraint) device on the forklift such as seat belts.
ESTABLISHMENT:  FIVE FORKLIFTS WERE NOT EQUIPPED WITH OPERATOR
RESTRAINT DEVICES.
SPECIFIC ABATEMENT DOCUMENTATION DEMONSTRATING THE ABATEMENT IS
COMPLETE MUST BE PROVIDED IN ACCORDANCE WITH 29 cfr 1903.19(D)(1).
