Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to entanglement, crushing, and/or struck by hazards due to the sudden and unexpected release of a manually lifted suspended load. a) VILLAGE COMMONS, SOUTH HADLEY MA WORK SITE; ON 11/17/2010, WORKERS ON A SCAFFOLD 32 FEET FROM THE GROUND WERE MANUALLY HOISTING A 248 POUND DEBRIS CHUTE WHICH WAS TO BE INSTALLED ON THE END FRAME OF THE SCAFFOLD. DURING THE MANUAL HOISTING OF THE CHUTE, THE CHUTE UNEXPECTEDLY DROPPED, EXPOSING FOUR WORKERS TO LIFTING LINE ENTANGLEMENT, CRUSHING AND STRUCK BY HAZARDS. AMONG OTHER METHODS, FEASIBLE AND ACCEPTABLE ABATEMENT METHODS TO CORRECT THESE HAZARDS INCLUDE: 1. FOLLOW THE MANUFACTURER'S INSTALLATION INSTRUCTIONS AND USE A SUITABLE HOIST TO INSTALL THE CHUTE. 2. USE A CRANE OR OTHER EQUIVALENT MECHANICAL LIFTING DEVICE TO INSTALL THE CHUTE.
