Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to [STATE HAZARD, E.G., CRUSHING HAZARD]: Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards while operating overhead cranes in that loads lifted by the crane were not secured by a hook latch. a)In the Tank Plant: On July 22, 2009 and at periods prior to, employees were exposed to struck-by and crushing hazards while operating overhead cranes in that loads lifted by the crane were not secured by a hook latch. In Accordance With (IAW) industry standards published by the American Society of Mechanical Engineers (ASME), B30.10, states that a hook with an inoperative hook latch is considered inoperative unless it is repaired or replaced. IAW ASME B30.16, "Hooks shall be equipped with latches unless the use of a latch creates a hazardous condition." Feasible and acceptable methods of abatement to correct this hazard include, but are not limited to following: a)Procure and attach a crane hook with an operable latch. b)Repair the current hooks by having a latch attached.
