Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm including injuries from stuck- by hazards to employees in that employees were exposed to swinging object hazards from metal lifting bars attached to hooks on a ASC Industries, 2 ton spreader bar that had damaged and missing safety latches.   a)        Vault Pouring Shop:  On or about September 25, 2018 and at times prior to, the employer exposed employees to struck-by  hazards and injuries from concrete vault hanging and being moved by spreader bar with hooks that had damaged and missing safety latches.
