recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of crushing injuries associated with
the rollover of a
lift trucks without the use of an operator restraint system:
(a)Fabrication/Weld Shop:  On January 26, 2010 and times prior to, an
employee
operated a Caterpillar fork lift (model # 422S) without the use of the
installed
seatbelts.
ABATEMENT NOTE: Among other methods, feasible and acceptable abatement
methods to
correct this hazard are:
1. To follow ASME B56.1-1993 Safety Standard for Low and High Lift Trucks
and require
the usage of an operator restraint system at all times.
2. To follow the manufacturers operating manual and wear the provided seat
belts at all
times when operating the lift trucks.
