29 CFR 1910.26(c)(3)(vii): Portable ladders were used for other uses than that which they were intended: (a) Abrasive Blasting Booth: On June 3, 2016 and times prior, an employee used a 12 foot Werner step ladder to open and close the top of the abrasive blasting booth. The step ladder was in the closed position and leaned against the side of the booth. The step ladder is designed to be used in the open and locked position.

29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of iron oxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 10 mg/m3: (a) Abrasive Blasting Booth: On June 3, 2016, an employee conducting abrasive blasting on carbon steel parts was exposed to iron oxide particulates (dust and fume) at an 8 hour Time Weighted Average of 30.15 milligrams per cubic meter of air for the 457 minute sampling period. The employee was exposed over the 8 hour Time Weighted Average of 10.0 milligrams per cubic meter of air listed in Table Z-1.

The employer has not implemented all feasible engineering or administrative controls to achieve compliance with the permissible exposure limit of 10.0 milligrams per cubic meter of air listed in Table Z-1.

29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter: (a) Abrasive Blasting Booth: On April 26, 2016 and times prior, an employee wears a 3M 7502 half-mask tight-fitting air purifying respirator while conducting filter change and cleanout of the abrasive blast booth dust collection system. The employer has not ensured that employee was fit tested prior to initial use of the respirator.
