Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to the employees, in that employees were permitted to trip out of the hole while methane was flowing out of the well and onto the rig floor: a.Well #2: Employees were permitted to trip drill string and bottom hole assembly out of the hole while methane was flowing out of the well and into the substructure and rig floor areas, exposing employees to a fire hazard as evidenced by an ignition, fire, and burns that occurred June 7, 2010. Among other methods, one feasible and acceptable abatement method to correct this hazard is when tripping out of the hole and pressurized methane gas is encountered, reinstall the air bowl rubber, when possible. If it is not possible to reinstall the air bowl rubber, employees are to be trained and instructed to shut the rig down, safely remove all ignition sources, and evacuate the area immediately. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED FOR THIS ITEM.
