Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to the employees in that employees were exposed to impalement hazards: (a)615 Cleveland Street work-site, Wayne: Employees were exposed to impalement hazards by falling onto form pins that were approximately 6 to 30 inches above the ground, as determined on 06/03/2010. NOTE: Among other methods, one or more feasible and acceptable abatement method to prevent this hazard is to; 1) Install caps over form pins, and 2) Install form pins to the level of the form brace. ABATEMENT CERTIFICATION REQUIRED FOR THIS ITEM
