Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to "crushing" injuries associated with operating a forklift
without seat belts:
a)Styro-Tech, LLC, 5005 Lima Street, Denver, CO:  On or about June 1, 2010,
the Nissan brand (Enduro 35) forklift was not equipped with seat belts.
Condition exposed the operator to injuries associated with falling out of
the
forklift or, crushing injuries associated with overturning the forklift.
