For the employee working in the derrick of the service rig, laying back
tubing being pulled from
the well on or about December 4, 2009.  The employer did not ensure the
employee was
provided with a secondary mean of escape from the tubing board.  Among
other methods, one
feasible and acceptable abatement method to correct the condition would be
to identify and/or
install a proper anchor points at the well site before or during rig up.
