Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, employees were exposed to steam explosions: a) On or about November 20, 2009, the employer did not ensure that employees were protected from steam explosion injuries while working with and near the Extruder where the door was not opened prior to trouble shooting the oil pump while the Extruder was in operation. One feasible and acceptable abatement method among others is to implement the manufacturer's manual operation and maintenance procedures. Pages 5-7 and 6-1. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").
