a) On or about November 20, 2009, the employer did not ensure that
employees were
protected from steam explosion injuries while working with and near the
Extruder
where the door was not opened prior to trouble shooting the oil pump while
the
Extruder was in operation.
One feasible and acceptable abatement method among others is to implement
the
manufacturer's manual operation and maintenance procedures. Pages 5-7 and
6-1.
