Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to electrical hazards due to improper installation
and use of
electrical equipment:
a)On or about January 12, 2010, and at times prior, Tagawa Greenhouse
Enterprises
did not ensure that employees were protected from electrical hazards.
Some instances
include, but are not limited to:
1)Missing covers and knock-outs throughout the ranges.
2)Inadequate strain relief on electrical cords throughout the ranges.
3)A non-GFCI plug next to the water fountain and on a boom in Range 2.
4)Electrical cords run through openings in walls.
5)Damaged extension cords throughout the ranges.
Among others, a feasible and acceptable method of abatement to correct
this hazard is to
establish a program that trains workers on the recognition of electrical
hazards, doing regular
inspections of electrical equipment, and performing appropriate
maintenance and repairs.

employees in that
employees were exposed to tripping and fall hazards:
a)On or about January 12, 2010, and at times prior, employees were exposed
to
trips
and falls in that a pit for the return water tank for a boiler was not
provided with rails or
adequate covers.
Among others, a feasible and acceptable method of abatement to correct
this hazard is to
ensure standard guardrails or covers are provided and installed.
