which were causing or likely to cause death or serious physical harm to
employees in that,
employees were subject to being struck-by or crushed by falling loads due
to inadequate
lifting devices:
a)  On or about April 7, 2010 and at times prior, employees were exposed
to crushing and
struck-by injuries in the event of falling loads due to damaged electrical
wires on the magnet
of the #2 overhead crane in the Tube Mill and the Bloomyard overhead crane
in the Rail
Mill.
Among other methods, a feasible and acceptable method to correct this
hazard is to
inspect and maintain these wires and adhere to the requirements of the
American
National Standards Institute (ANSI/ASME) B30.20- Below the Hook Lifting
Devices.
