Section 5(a)(1) of the Occupational Safety and Health Act of 1970; The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, employees were exposed to a structural collapse of the work-over rig. a) On or about June 10, 2010, and at times prior, the employer did not ensure that employees were protected from crushing hazards due to structural collapse while working with and near the work-over rig #9628. The guy wires were not properly tensioned according to the manufacturer's or API Practice 4G specifications. Among other methods one feasible and acceptable abatement method to correct this hazardous condition would be to develop and implement procedures for inspection, maintenance, and repair of drill rig structures that conforms to API Recommended Practice 4G, Third Edition, April 2004. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").

The deadman anchor on the south- west side was not pull tested after it was runover by a truck. The anchor sheave was damaged. Among other methods one feasible and acceptable abatement method to correct this hazardous condition would be to develop and implement procedures for inspection, maintenance, and repair of drill rig structures that conforms to API Recommended Practice 4G, Third Edition, April 2004. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").
