employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees, in that employees
operating chain hoists were exposed to struck-by hazards, due to the chain
hoists not being
marked with their load ratings.
a.Republic Engineered Products, Inc. located in Lorain, Ohio:  On or about
June 24, 2010,
the employer did not ensure the chain hoist used to place a 200 pound hub
onto the
spindle for Cold Saw #1, located in the Conditioning Building, was marked
as to its load
rating.
b.Republic Engineered Products, Inc. located in Lorain, Ohio:  On or about
June 24, 2010,
the employer did not ensure the chain hoist used to place a 200 pound hub
onto the
spindle for Cold Saw #2, located in the Conditioning Building, was marked
as
to its load
rating.
Among other methods, a feasible abatement method to correct this hazard is
to comply with the
requirements of ASME B30.16-2007 "Overhead Hoists" Overhead, Section
16-1.1: Marking.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation (
using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition,
documentation demonstrating that abatement is complete must be included
with your
certification.  This documentation may include, but is not limited to,
evidence of the purchase
or repair of the equipment, photographic or video evidence, or written
records.

suspended chain hoist to the
jib crane at the #1 cold saw, was bent.
a.Republic Engineered Products, Inc. located in Lorain, Ohio:  On or about
June 24, 2010,
the employer did not ensure the hook used to secure the chain hoist to the
jib crane on
Cold Saw #1, located in the conditioning building, was in good working
order.  The
hook was bent open, allowing the weight of the chain hoist and attached
load to rest on
the tip of the hoist hook.  The chain hoist was used to maneuver a 200
pound abrasive
wheel hub from the saws spindle.
Among other methods, a feasible abatement method to correct this hazard is
to comply with the
requirements of ASME B30.10-2005 "Hooks", Section 10-1.2.3 Maintenance.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation (
using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition,
documentation demonstrating that abatement is complete must be included
with your
certification.  This documentation may include, but is not limited to,
evidence of the purchase

were exposed to crushing injuries from overhead cranes that had
malfunctioning bridge brakes
and hoist controllers.
a.Republic Engineered Products, Inc. located in Lorain, Ohio:  On or about
May 13, 2010,
the employer did not ensure the #4 wire crane located in the Conditioning
Building was
equipped with functioning bridge brakes.  The brakes were not allowing the
crane to
stop.
b.Republic Engineered Products, Inc. located in Lorain, Ohio:  On or about
May 13, 2010,
the employer did not ensure the #4 wire crane located in the Conditioning
Building was
equipped with a properly functioning hoist controller. The hoist
controller allowed loads
to be dropped by the hoist.
c.Republic Engineered Products, Inc. located in Lorain, Ohio:  On or about
May 13, 2010,
the employer did not ensure the #3 wire crane located in the Conditioning
Building was
equipped with functioning bridge brakes.   The bridge brakes were not
allowing the crane
to stop.
Among other methods, one feasible and acceptable method to correct this
hazard is to ensure
cranes are removed from service when deficiencies are identified in their
safe functioning
according to ASME B30.2-2005 Overhead and Gantry Cranes (Top Running
Bridge, Single or
Multiple Girder, Top Running Trolley Hoist).
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation (
using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition,
documentation demonstrating that abatement is complete must be included
with your
certification.  This documentation may include, but is not limited to,
evidence of the purchase
or repair of the equipment, photographic or video evidence, or written
records.

general duty clause of the Occupational Safety and Health Act, for under
the hook lifting devices
not being marked as to their capacity, which was contained in OSHA
inspection #311492201,
Citation #1, Item #1 and was affirmed as a final order on July 19, 2009,
with respect to a
workplace located at 1807 E. 28th St., Lorain, Ohio.
