recognized hazards that
were causing or likely to cause death or serious physical harm to
employees, in that a workplace
hoist was not in good repair, as the hoist hook was not equipped with a
throat latch.
a) Alco Manufacturing Corporation, LLC, Elyria, Ohio:  On or about January
26,
2011, the employer did not ensure that the hook on the overhead 3 Ton
Shaw-Box
top-running, underhung crane, located in the steel receiving area, was
equipped
with a throat latch, exposing employees to struck-by and crushing hazards
should
the load being lifted roll out of the hook.
Among other methods, one feasible and acceptable method to correct this
hazard is to comply
with the requirements of American Society of Mechanical Engineers (ASME)
B30.16-2007
"Overhead Hoists (Underhung)", Section 16-1.2.9 "Hooks".
In accordance with 29 CFR 1903.19(c), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
