29 CFR 1910.147(f)(1): The sequence of actions required by 29 CFR 1910.147(f)(1) when testing or positioning of machines, where temporary removal of lockout or tagout devices from the energy isolating device is required, were not followed. ALCO Manufacturing Corp., LLC - Elyria, OH: On or about April 19, 2017, the employer did not develop and utilize a procedure to protect employees during troubleshooting/testing activities. Employees troubleshooting a coolant leak on the Amada CNC machine (model S-10; s/n 131) were exposed to pinch point and rotating part hazards in that the following sequence of actions were not implemented: - Machine cleared of tools. - Employees removed from hazardous areas around the machine. - Machine energized with effective employee protecting while testing or positioning; and - Isolate the machine from energy source(s) in accordance with paragraph (d) of this section and continue servicing/maintenance activities. ALCO Manufacturing Corp., LLC was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1910.147(d)(2), which was contained in OSHA inspection number 1227935, citation number 1, item number 3b and was affirmed as a final order on October 26, 2012, with respect to a workplace located at 10585 Middle Ave., Elyria, OH 44035.
