were causing or likely to cause death or serious physical harm to

were exposed to the hazard of falling to a lower level:
(a)Wooden Deck outside of Showroom Building - An employee used an 8-foot
Husky fiberglass stepladder to access the roof of the Showroom Building to
spray
bee/wasp nests on the eave of an adjacent building.  The employee stepped
on the
top step of the ladder to access the roof, on or about August 2, 2010.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard includes using a ladder of sufficient height which does not require
an employee
to step or stand higher than the step or rung indicated on the
manufacturer's label
marking the highest standing level of the ladder and which does not
require an employee
to step or stand on the top step of a step ladder as specified in the
American National
Standard for Ladders - Portable Reinforced Plastic - Safety Requirements
(ANSI A14.5-
2007), Section 9.3.2 Climbing and Working Locations.
