Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a discharge conveyor which was not equipped with emergency
stops, limit
switches, or pull cords:
(a)R.H. Sheppard Company, Incorporated, 447 East Middle Street, Hanover,
PA,
Old Foundry - Two (2) maintenance employees were cleaning sand out of the
discharge conveyor (Machine ID F202/No model #/Serial # D-510-3075) to get
the belt on track. The employer did not ensure that the equipment was
equipped
with an emergency stop, limit switches, or a pull cord. The machine was
turned
on while they were pulling excess sand out of the conveyor belt with their
hands
and arms.  One (1) employee was close to the tail pulley and got his left
hand
caught in the belt, causing an arm amputation on or about May 23, 2011.
Among other methods, one (1) feasible and acceptable method to correct
this hazard is
to provide emergency stop buttons, pull cords, limit switches or similar
emergency stop
devices, in accordance with ASME B20.1 (2009), Section 5.11.2(c).
Abatement certification and documentation required within 10 days after
abatement date.
The certification shall include a statement that abatement is complete,
the date and
method of abatement, and state that employees and their representatives
were informed
of this abatement. Abatement documentation shall include documents
demonstrating that
abatement is complete, such as evidence of the purchase or repair of
equipment,
photographic or video evidence of abatement or other written records.
