Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish to each of his employees employment and a
place of employment
which were free from recognized hazards that were causing or likely to
cause death or serious
physical harm to employees in that employees were exposed to:
On or about 9/7/2010, employees were exposed to the hazard of explosion
from combustible
paper dust contained within duct work from the shredder through the baler
and into the dust
collectors.  A process hazard analysis had not been conducted and
documented to determine the
associated fire and explosion hazards and the necessary safety provisions
to prevent fire and
explosion.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to follow and implement NFPA 654, Chapter 4.2., Process Hazard Analysis,
which requires the
design of fire and explosion safety provisions shall be based on a process
hazard analysis of the
facility, the process, and the associated fire and explosion hazards.
PLEASE NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS
REQUIRED FOR THIS VIOLATION.
