a. At the maintenance department, an employee was exposed to caught by
and struck by hazards while working with a lathe (Birmingham, Model
CT-144G) that lacked a provision to prevent automatic restart upon
restoration of power.
b. At the maintenance department, an employee was exposed to caught by
and struck by hazards while working with a milling machine (Bridgeport,
Model 8F) that lacked a provision to prevent automatic restart upon
restoration of power.
c. At the maintenance department, an employee was exposed to caught by
and struck by hazards while working with a milling machine (Ultima,
Property #111003) that lacked a provision to prevent automatic restart
upon restoration of power.
d. At the maintenance department, an employee was exposed to caught by
and struck by hazards while working with a drill press (Walker-Turner,
Serial #111017) that lacked a provision to prevent automatic restart upon
restoration of power.
e. At the maintenance department, an employee was exposed to caught by
and struck by hazards while working with a lathe (Enco, Model 111-1450)
that lacked a provision to prevent automatic restart upon restoration of
power.
Among the feasible and useful methods to correct the hazard is installing a
magnetic switch to prevent automatic restart upon restoration of power
after a
power failure.
