On or about August 2, 2010, throughout the facility, the employer failed
to anchor racks to the
floor with anchor bolts and replace damaged racks.  Employees were exposed
to being struck
by collapsing storage racks.
One feasible and acceptable abatement method to correct this hazard
includes but is not limited
to: develop and implement a program to periodically inspect the storage
racks and ensure
damage is promptly repaired in accordance with ANSI MH16.1:2008, Section
1.4.9 Rack
Damage.  In the event the racks are being installed or moved, care should
be taken to properly
anchor them.r
