were exposed to THE HAZARD OF BEING STRUCK-BY THE LOAD SHOULD THE HOIST
FAIL:
a)Shell Sand area, on or about 09/07/2010: A Gardner Denver, 2-ton overhead
pneumatic hoist was being operated in the foundry without a safety latch on
the hoist hook and no frequent daily inspection had been performed by an
operator or other designated person.
b)Sand Molding area, on or about 09/07/2010: A Yale 5-ton overhead
pneumatic hoist was being operated in the foundry without a safety latch on
the hoist hook and no frequent daily inspection had been performed by an
operator or other designated person.
c)Impregnation cell area, on or about 09/07/2010: A Coffing 1/2-ton
overhead
pneumatic hoist was being used in the foundry without a safety latch on the
hoist hook and no frequent daily inspection had been performed by an
operator or other designated person.
Among other methods, several feasible and acceptable abatement methods to
correct
this hazard are to:
1.Repair or replace the safety latches and ensure they meet the
requirement of
the ASME B30.16-2007, Overhead Hoists, Section 16-.1.2.9.
2.Conduct frequent daily inspections during operation of the of the hoists
for
missing or defective equipment in accordance with ASME B30.16-2007,
Overhead Hoists, Section 16-2.1.1 Inspections.
Abatement certification must be submitted for these items.
