were causing or likely to cause death or serious physical harm to

were exposed to the following:
a.At the establishment, an employee was exposed to caught between hazards
when the
orange hydraulic transmission jack he was using when working on a
semi-truck
transmission estimated to weigh approximately 1,000 pounds unexpectedly
lowered. The
company's investigation found that the jack lowered when the transmission
jack's handle
struck the hydraulic release valve. The transmission jack was put back
into service
without abatement of the caught between hazard.
Among other methods, feasible and acceptable methods to correct the
hazards are to
follow, "The American Society of Mechanical Engineers, "Safety Standard
for
Portable
Automotive Lifting Devices" (ASME PALD-2009).

damaged 12 foot
fiber glass step ladder to access the tops of semi-truck trailers. Two of
the damaged fiber
glass step ladder's side rails had been repaired using pieces of angle
iron placed on the
insides of both side rails and bolted in place.
Among other methods, one feasible and acceptable method to abate this
hazard is to
follow ANSI-ASC A14.5-2007, "American National Standard for
Ladders-Portable
Reinforced Plastic-Safety Requirements".
